Intake forms prior to our initial consultation:
~Initial Interview Questionnaire-this provides me with an overview of your goals, your health history, and a better understanding of the environment and stressors you grew up around and currently live with
~Nutritional Assessment Questionnaire (NAQ)-320 question symptom analysis which clues us into how your various organs and systems are functioning and which areas of the body need the most support
~5-7 day Food and Mood Journal-perhaps the most important initial step as this self-tracking allows you as well as me to gain awareness around your current food habits and symptoms, and as we know, awareness is the first step toward change
3 or 6 month package:
First month-
~60-90 minute initial call-we get to know each other, thoroughly review and evaluate your intake forms, clarify your health and wellness concerns, and define your goals
~30-60 minute recommendations followup call-we work together to clearly lay out your short and long-term goals, review findings from the initial consult, and go over your initial functional nutrition, movement, and mindfulness recommendations
Following 2-4 months-
~2 60 minute coaching calls per month
~NAQ Reassessment to track progress and symptoms as time progresses (once for 3 month package, twice for 6 month package)
~Customized PDFs including grocery guides, digestible education, nutrition and mindfulness tips, plate building, exercises, and more
~Individualized three-pillared holistic health plan (nutrition, movement, mindfulness) updated every two weeks
~Targeted top-quality supplement recommendations and plan through Fullscript when needed
~Unlimited messaging and voice note access to me during business hours via Voxer

A la carte (although I highly recommend 3 months minimum work together to achieve long-term results)-$275 initial intake, call and recommendations and $150 for followup calls
Above is an example of the symptom burden graph generated from a client’s initial NAQ. As you can see, based on her answers and the symptom analysis, sugar handling is of the highest priority, so I started my protocol with her focused on providing recommendations that would help her to rebalance her blood sugar. This consequently helped her restore her energy, balance her hormones, improve her sleep, and lower her stress levels.

2 week reset
Virtual Group Program

structure
This is a two-week nutritional and lifestyle reset to start off your 2023 with intention, clarity, and peace.
When you join the program, you will gain access to our private Facebook group where I will be posting daily videos and exercises to complete. We will work on weaning off of the common inflammatory culprits of sugar, alcohol, and caffeine in the week leading up to January 16th, and I will provide you with materials on building a healthy macronutrient (protein, fat, carbs) balanced plate and a sample grocery list. We will have our group kickoff call on Monday January 16th. Over the two weeks, you will cook as much of your own food as possible. You will eat three nourishing meals made up of real, whole foods a day, support your detoxification processes with homemade teas and elixirs, and take time each day to get quiet with yourself and reflect on what you want to create in the year to come. I’ll provide you with daily journal prompts as well as guided yoga and meditation, and I’ll provide suggestions for lifestyle shifts to incorporate over the two weeks to help renew your energy, like screens off after 8pm and in bed by 10pm!
We’ll have a small community via the FB group to support and encourage each other as we take this time to recharge, look within, and establish healthy nutrition and lifestyle routines for 2023. In the week following the 2-week reset, I will provide support and guidance on how to reenter your normal routine and social life while holding onto healthy habits!

Day 01 / Looking within
The first week is all about you, and in the second week we shift our focus to others in your world. On day 1, you’ll take a deep look at yourself and reflect on where you are today and what has worked for you and given you energy over the past year. I’ll lead you in a gratitude meditation, and you’ll find acceptance for where you are at in this moment.

Day 02 / Letting GO
On Day Two you’ll reflect on what you have struggled with over the last year, and on the habits, mindset, beliefs, and actions that have drained your energy. Once you have pinpointed what hasn’t been working for you, we will work on letting go of what you do not want to carry with you into the next year.

Day 03 / Setting intentions
On Day Three, you’ll envision how you want your life to look. You’ll allow yourself to see what you want as tangible and even inevitable, and we’ll clarify and proclaim our desires so that we can set intentions for the next year with a plan for how to get there.
